Job description: - This is for full-time position - This position will cover Ocala, and the surrounding area. - Works indoors in Agency office and patient homes and travels to/from patient homes. RESPONSIBILITIES: 1. Understands and adheres to established policies and procedures. 2. Implements the nursing care plan for each patient. 3. Initiates preventive and rehabilitative nursing procedures as appropriate. 4. Relays observed signs and symptoms and reports to the physician and Clinical Manager reactions to treatments, including drugs and changes in the patient's physical or emotional condition. 5. Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home. 6. Evaluates with nurses and therapist the effectives of the nursing/therapy service to the patient and family under their care. 7. Maintains accurate and complete records of observations, treatments and care of patient. 8. Participates in medical record audit as assigned. 9. Attends staff meetings, patient care conferences and in-services as scheduled. 10. Takes on-call duty, nights, weekends and holidays as assigned. 11. Is responsible for: · Submitting any changes in schedule to Director of Clinical Services/Nursing Supervisor on a daily basis. · Participating in patient care conferences to discuss the need for involvement of other members of the health team, such as physical therapist or speech language pathologist. 12. Maintains proper documentation and regularly updates the chart. 13. Assists other LPNs and RNs in performing specialized procedures. 14. Prepares equipment and materials for treatments. 15. Receives report from field clinicians and coordinates with physicians, other disciplines and other staff members. 16. Assists the patient in learning appropriate self-care techniques. 17. Provides administrative support to clinical management/clinicians in various patient-related concerns. 18. Assists in conducting case conferences with clinicians 19. Coordinates/sends order orders to doctors’ offices. 20. Assists in coordination of patient scheduling. 21. Performs other related tasks that may be assigned from time to time Qualifications and Requirements: - Valid Florida Registered Nurse (RN) / LPN license -Must have a valid FL Driver's License - Must have current CPR Card - Must have current AHCA Background Screening Report -* Must have either Chest XR report, 2 Step TST report, orTB Lab Test Report* (Quantiferon Gold) - Must have a Doctor's Certificate of Good Health - Strong knowledge of home health regulations and best practices - Excellent clinical assessment and critical thinking skills - Ability to work independently and manage time effectively - Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members - Proficient in using electronic medical record (EMR) systems Work Location: In person
