Job Summary: The Registered Nurse in the Transitional Care Unit at The Ottawa Hospital will provide high-quality nursing care to patients transitioning between acute care and home or other care settings. This role is critical in ensuring smooth patient transitions and continuity of care. Key Responsibilities: - Assess, plan, implement, and evaluate patient care in the Transitional Care Unit. - Collaborate with interdisciplinary teams to develop and implement individualized care plans. - Monitor patient progress and adjust care plans as necessary. - Educate patients and families on post-discharge care and resources. - Maintain accurate and timely documentation of patient care. - Adhere to hospital policies, procedures, and regulatory standards.
