Job description: - This is for a PRN and full-time position This position will cover Clermont, Lake County, The Villages, and the surrounding area. - Provide direct patient care in a home health setting - Perform comprehensive assessments of the patient's health status - Develop and implement individualized care plans - Administer medications and treatments as prescribed by physicians - Monitor and evaluate the patient's progress and make necessary adjustments to the care plan - Educate patients and their families on managing their health conditions - Coordinate with other healthcare professionals to ensure continuity of care - Document all patient interactions and interventions accurately and timely -Attend case conferences to provide and get input from other healthcare professionals to ensure the best possible care for patients Qualifications and Requirements: - Valid Florida Registered Nurse (RN) license -Must have a valid FL Driver's License - Must have current CPR Card - Must have current AHCA Background Screening Report -* Must have either Chest XR report, 2 Step TST report, orTB Lab Test Report* (Quantiferon Gold) - Must have a Doctor's Certificate of Good Health - Strong knowledge of home health regulations and best practices - Excellent clinical assessment and critical thinking skills - Ability to work independently and manage time effectively - Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare team members - Proficient in using electronic medical record (EMR) systems Work Location: On the road
